  • Primary care leader for physicians and APCs driving quality improvements, streamlining coverage, and team building

  • Led a team of urgent care clinicians to provide high quality primary and urgent care to patients in the Marin district with NPS scores and patient feedback consistently over expected benchmarks

  • Managed a team consisting of a geriatrician, social worker, designer, product designer, and EPIC software developer, and liaised with the CMIO on implementing $150,000 Samuels Foundation grant funding for “Enhancing FQHC Services for Older Adults”

  • Reduced time for chart reviews on geriatric patients from 20 minutes to 5 minutes by designing an EHR snapshot tool based on the “5 M” Model of geriatric care

  • Reorganized workflows in collaboration with Care Management to streamline clinical care so that elderly patients have improved and well documented end of life planning, access to durable medical equipment, and improved medication reconciliation for polypharmacy
